Subscribers Being Double Charged After Two Subscriptions Are Created

Hello,
We’re encountering more and more subscribers are having two accounts be created after signing up for once. These double subscriptions are resulting in double charges and making it difficult to edit their standing on the muvi back end.
Please advise, thank you.
